Death Certificates

Certified copies of Death Certificates are available from the office of:

You may obtain a certified copy of a Death Certificate by providing the following:

1. Your full name with proper spacing.
2. Exact name of the person for whom you are seeking the record (or exact name on record).
3. Date of death
4. Place of death
5. A photocopy of your Photo I.D. (i.e. Drivers License or Military I.D.)

The cost is $12.00 for the 1st copy and $6.00 for additional copies of the same record. Your check or money order should be made payable to "Madison County Clerk".

Federal Express - $17.50 plus cost of record
Credit Card - $7.00 plus cost of record

Individuals must be 18 years or older in order to obtain a death record.
