Birth Certificates

Certified copies of Birth Certificates are available from the office of:

You may obtain a certified copy of a Birth Certificate by providing the following:

  1. Your full name with proper spacing.
  2. Exact name of the person for whom you are seeking the record (or exact name of a child on record).
  3. Date of birth
  4. Place of birth
  5. Father´s Name
  6. Mother´s Name
  7. A photocopy of your Photo I.D. (i.e. Drivers License or Military I.D.)

The cost is $10.00 for the 1st copy and $2.00 for additional copies of the same record. Your check or money order should be made payable to "Madison County Clerk".

Birth records cannot be faxed

Individuals must be 18 years or older, except for a minor mother, in order to obtain a birth record.

Certified copies of the record will only be issued if the requestor´s name appears on the Birth Record, which could be either as a parent or as the child.
